If you have received CEs through Behavioral Tech in the past, and you want to request a reprint of your missing or lost document, please fill out the form below, and pay the $5 non-refundable CE copy request fee.

The administrative fee will be charged ONE TIME when your CE is locate. Your certificate will be sent to your email address.

Administrative fees are $195 for CEs earned before the year 2017 and $45 for 2017 or later.
